Hedgehog Pathway Inhibitor
Itraconazole is an antifungal, similar to ketoconazole. Ketoconazole has been used to suppress the enzymatic production of androgens in the adrenal glands. Unlike ketoconazole, itraconazole is a potent suppressor of the Hedgehog Signaling Pathway that cancers may use for growth. It is currently finding a use in reversing basal cell carcinoma. A small study last year compared low dose to high dose itraconazole in men with metastatic PC, and found some activity. Because it potently suppresses the CYP3A4 enzyme, it has many drug interactions and may exhibit hepatotoxicity.
