Last week the CHAARTED study results were published: Reference: E3805: CHAARTED: ChemoHormonal Therapy Versus Androgen Ablation Randomized Trial for Extensive Disease in Prostate Cancer. The full protocol for this trial can be found at clinicaltrials.gov/ct2/show/NCT00309985. Currently it is not clear how all current trials out there for these patients will be affected. There are many trials out there that are testing Enzalutamide, Abiraterone Acetate, TAK700 and more in this setting. At SWOG there has been special session to discuss though no official modifications have been made. But it is quite possible that many trials may face recall or modification to allow for chemotherapy early. Of course trials are still needed to see if there is a better approach than docetaxel as it is rich in side effects. What happens moving forward will be interesting. Tony

I agree Tony. Today a friend from Turkey emailed about a 33 yo who presented with nodes and painful bone mets. Also a few small liver lesions. I recommended that he start chemotherapy ASAP. I've recommended this intermittently over the years especially in young guys. All have remained in long remissions!

Wow, 33yo. What can I say? That is just too young for metastatic cancer.
It's great to hear that early chemotherapy is showing such promise. I know in our meeting last week I could hear positive excitement in all the committee members on this trial.
Thank you Dr. Vogelzang for joining our Scientific Review Board here. I know my friend that is helping me here will be excited to see such great support in this effort.
Now to keep herding the other great advocates into the tent.

Dr. V. / Tony:
I've been off Hormone Therapy for over a year now and even though I am currently undetectable, I am realistic enough to know that I will soon have to go back on ADT. In an intermittant HT setting, such as me, is chemotherapy + HT a valid option now. I'm game for anything.
(Testing signature as well...hopefully it shows once I post.)

Jerry I will forward to Dr. V. He didn't sign for an account but rather posted as a guest. My plan is to turn off guest posting as it is not secure. For now I am allowing it before mandating it. I will ask him a "hypothetical" question using your data.
Your signature looks great. You are outside of the CHAARTED recruitment criteria but that does not mean you would not benefit. You were just was not the question asked in that trial. If anything you would have been in the control arm.
