Interesting new study of men on AS… This study found a correlation (an association, not causality) between lower levels of free testosterone and an increased likelihood of “upstaging” for low-risk men following an Active Surveillance program. “Upstaging” usually leads to an end of the AS protocol, and movement to curative active treatment. At this point, we don’t know if (a) the PC may reduce the free testosterone concentrations, or if (b) the low free testosterone my contribute to the more aggressive PC—which one may “cause” the other—we only know that there is a statistically significant correlation between the two. This seems like a good “baby-step” toward the continuously improving differentiation between men best suited for AS programs. The way I read this is that if you are one of the thousands and thousands of men annually who fit the criteria for AS, a high free testosterone may portend to a longer stay on an AS protocol...
